To become a participating provider, please complete the necessary paperwork (credentialing document, credentialing checklist and W-9), so that we can include your office as an in-network, contracted provider. Please contact the Network Development department to obtain the appropriate contracting documents for your provider type by e-mailing becomeaprovider@wellcare.com.

If you are contracting with WellCare for the first time, your contracting paperwork will be provided by e-mailing the Network Development department at becomeaprovider@wellcare.com.If you are re-contracting with WellCare, please call the Provider Hotline at 1-866-231-1821
(TTY/TDD: 1-877-247-6272), select option 3 and then option 6 and request to have a Provider Relations representative contact you.

Notice to all Primary Care Physicians

A current Medicaid identification number is required for each location where services will be rendered to Medicaid/PeachCare for Kids beneficiaries.
