Dear Provider,

Effective July 1, 2009, the WellCare Pharmacy & Therapeutics Committee has selected Levemir® as the preferred long-acting basal insulin on the PDL. Lantus® will be moved to non-preferred status.  

Members that are currently using Lantus® will be grandfathered and continued on this medication through December 31, 2009. All members that require a new prescription for long-acting basal insulin should be tried on Levemir® first.
