Dear Provider,

 

Effective October 28, 2010, WellCare Health Plans, Inc. will no longer accept handwritten or replicated claim form submissions. While paper claims will continue to be accepted, going forward they must be "red," meaning they must be submitted on original CMS-1500 or UB-04 forms.
